This role involves working within the @Home service. The service provides a rapid response to patients in the community who have an exacerbation of medical problems or following a fall. Referrals are from GPs to an MDT (Consultants, ANPs, Physio and OT). Consequently potential candidates must be confident and competent in working in a fast-paced community environment (although some clinic sessions are also provided). You must be confident working with patients with acute medical problems and be able to drive and have access to transport.
